Full-Time or Part-Time Physical Therapist or Physical Therapy Assistant

We are hiring for both full-time and part-time positions. Join our team to provide exceptional home care physical therapy services.

Qualifications:

• Physical Therapy License or Physical Therapy Assistant License (Required)
• CPR Certification (Preferred)

Pay:

• Physical Therapist: $42.00 - $60.00 per hour DOE
• Physical Therapy Assistant: $30.00 - $35.00 per hour DOE

Primary Responsibilities:

• Provide physical therapy to home care patients and manage cases when indicated.
• Ensure timely and accurate communication and documentation of services.
• Promote and develop physical therapy services in home care.

Specific Job Duties:

• Evaluate and treat patients with physical disabilities due to injury, disease, or surgery in a home care setting.
• Perform initial assessments and related documentation.
• Develop, revise, and execute care plans for restorative and maintenance patients.
• Collaborate with healthcare team members to ensure consistent care.
• Monitor patient progress and document accordingly.
• Educate patients and caregivers on the use of medical and adaptive equipment.
