Job summary

LTC Therapy Services in Harwich, MA is seeking a full-time Occupational Therapist to join our skilled nursing facility team. You will assess residents, develop therapy plans, and deliver interventions focused on ADLs, coordination, balance, cognition, and upper-extremity strength.

You will collaborate with PT, SLP, nursing, and physicians to achieve interdisciplinary goals while educating residents, families, and caregivers on adaptive strategies and safety techniques.

Qualifications

• Active OT license required.
• Excellent communication and teamwork skills.
• Previous SNF experience is a plus.

Responsibilities

• Assess residents' functional abilities and develop individualized therapy plans.
• Deliver treatment interventions focused on ADLs, coordination, balance, cognition, and upper extremity strength.
• Educate residents, families, and caregivers on adaptive strategies and safety techniques.
• Work closely with PT, SLP, nursing, and physicians to support interdisciplinary care goals.
• Track resident progress and maintain accurate documentation.
