Timberwood Nursing & Rehab Center is seeking a dedicated Occupational Therapist for a full-time role in Livingston, TX. Join our in-house therapy team and contribute to a dynamic environment while providing meaningful care and facility-level decision support.

Qualifications

• Licensed Occupational Therapist required.
• Open to applicants of all experience levels.

Responsibilities

• Screen/evaluate patients with cognitive and ADL functioning disorders and develop care plans.
• Use standardized assessments and evidence-based practice to support interventions.
• Provide rehabilitative, skilled, and medically necessary treatment.
• Administer procedures as part of the rehabilitation plan (ADL retraining, exercises, modalities).
• Assess progress and adjust treatments in the clinical record.
• Evaluate outcomes and modify services as needed.
• Provide counseling to patients and families.
• Communicate progress with supervisor and health team.
• Provide comprehensive discharge summaries.
