Renewal Rehab is seeking a full-time Occupational Therapist to join a skilled nursing facility team in Illinois. You will evaluate and treat patients, develop individualized care plans, and collaborate with therapists to deliver high-quality outcomes.

Qualifications

• Must be licensed Occupational Therapist to apply.

• Create and implement personalized care plans.
• Collaborate with team to ensure quality outcomes.
• Maintain documentation to meet regulatory requirements.
• Obtain and update physician orders.
