Job summary

Ganadonr is seeking a dedicated Occupational Therapist to join our in-house team, focusing on resident independence, safety, and quality of life through meaningful daily activities and functional outcomes.

You will assess and treat patients, develop individualized plans for ADLs/IADLs, and collaborate with an interdisciplinary team to ensure high-quality care, in a SNF or long-term care setting. A Texas OT license is required, and Net Health EHR experience is a plus.

Qualifications

• Active and valid Texas Occupational Therapy license.
• SNF or long-term care experience preferred.
• Knowledge of occupational therapy treatment practices.

Responsibilities

• Consult with patients to evaluate their condition and impact on daily living.
• Develop individualized treatment plans to maximize ADLs/IADLs.
• Assess home environments and recommend modifications.
• Provide adaptive equipment training to support independence and safety.
• Deliver treatments per the plan of care and monitor response.
• Collaborate with the interdisciplinary team to ensure high-quality care.
• Complete documentation in compliance with policies and regulations.
• Attend required meetings with the Director of Rehab.
