Job summary

Crossroads Treatment Centers is seeking a Nurse Practitioner to join our outpatient MAT clinics. You will evaluate and manage patients with opioid use disorder, prescribe appropriate treatments, and document care per our standards.

You will collaborate with the team to coordinate holistic patient services and perform injections as needed. This is a full-time, on-site position with weekly hours around the 34-hour mark, offering comprehensive benefits and a supportive work environment within

• Certified Nurse Practitioner licensed in the state of practice.
• DEA licensure in the state where the center is located.
• Additional certificates or education may be required depending on state rules.

Responsibilities

• Evaluate and manage patients with opioid use disorder in outpatient settings.
• Prescribe medications and treatments as indicated by assessment.
• Complete medical documentation per Crossroads standards.
• Collaborate with clinic staff to provide holistic patient care and referrals.
• Perform injections as part of patient care.

_ Day in the Life of a Nurse Practitioner _

• Evaluate and manage patients with opioid use disorder in our outpatient clinic settings

• Prescribe appropriate medications and treatments as indicated by patient assessment

• Complete medical documentation per Crossroads standards

• Collaborate with clinic staff to provide holistic care to patients, including referrals to other medical providers and community services

• Will perform injections.

_ Schedule & Locations _

• Expected weekly hours: 34

• Primary center: Sharon Hill

• Proposed schedule:

• Week 1: Mon-Off, Tues Sharon Hill 7am-12pm drive to Norristown 1pm-7pm, Wed Off, Thurs Sharon Hill 8-6, Fri Sharon Hill 8am-3pm, Sat Kensington 7:30am-2pm; Sun Kensington 7am-12pm

• Week 2: Mon-Off, Tues Sharon Hill 7am-12pm then drive to Norristown work 1pm-7pm; Wed off, Thurs Sharon Hill 8-6, Fri Sharon Hill 8am-3pm, Sat off, Sun off

_ Education and Licensure Requirements _

• Certified as a Nurse Practitioner in the state you are applying to work in

• DEA licensure in the state of the center

• Additional education or certification may be required depending on state of practice
