Job summary

Avamere Three Fountains in Medford, Oregon is seeking a Licensed Practical Nurse to provide essential nursing care under RN supervision. The role includes measuring vital signs, administering medications, wound care, hygiene support, and aiding residents with daily activities.

The LPN will monitor residents, document changes, and communicate effectively with families and health professionals. Experience in long-term care and EMR documentation is preferred.

Qualifications

• Must have a nursing degree from an accredited college or university.
• Must hold an active LPN license in the state.
• Current CPR/BLS certification.
• Experience in long-term care, geriatric rehab, sub-acute, or critical care settings preferred.
• Experience with EMR documentation.

Responsibilities

• Provide basic nursing care under RN supervision, including vital signs, medications, wound care, hygiene, mobility and nutrition support.
• Record patient history and monitor condition; communicate changes to RN.
• Collaborate with CNAs and other health professionals to ensure quality resident care.

Licensed Practical Nurse

Status/Shift: Full Time Day Shift & Full Time Noc Shift Available

Location: Avamere Three Fountains: 835 Crater Lake Ave, Medford Oregon

Job Summary

Licensed Practical Nurse provides basic nursing care, including vital signs, medication administration, wound care, and assisting residents with daily activities, under the supervision of a Registered Nurse (RN)

support and comfort to residents and their families.

Essential Duties and Job Responsibilities

• Basic Nursing Care:
• Vital Signs:Accurately measure and record vital signs like temperature, pulse, blood pressure, and respirations.
• Medication Administration:Administer medications as prescribed by a physician, following facility protocols and under the supervision of an RN.
• Wound Care:Perform wound care, including changing dressings and monitoring for signs of infection.
• Hygiene and Mobility:Assist residents with personal hygiene, dressing, bathing, and mobility.
• Nutrition and Hydration:Ensure residents receive proper nutrition and hydration, assisting with feeding if necessary.

Patient Monitoring:

• Patient History:Record a patient's history and current condition.
• Observation:Observe and document changes in a resident's condition, reporting any concerns to the RN.
• Communication:Communicate effectively with residents, families, and other healthcare professionals.

Collaboration and Supervision:

• RN Supervision:Work under the supervision of an RN, following their guidance and instructions.
• CNA Supervision:Supervise certified nursing assistants (CNAs) in providing basic nursing care.
• Teamwork:Collaborate with other healthcare professionals to ensure residents receive comprehensive care.

Other Duties:

• Patient Safety:Maintain a safe environment for residents, following infection control protocols and other safety guidelines.
• Documentation:Accurately document all care provided, including medications administered, vital signs, and any observations.
• Emotional Support:Provide emotional support and comfort to residents and their families.

• Must possess a nursing degree from an accredited college or university.
• Must possess a current, unencumbered, active license to practice as an LPN in this state.
• Must maintain and have an active CPR/BLS during employment.
• Prefer experience in long term care, geriatric rehab, sub-acute and or critical care clinical setting.
• Experience in electronic medical records documentation.
