Job summary

Ativoyuma in Yuma, AZ is looking for a Care Partner (MC) to assist Memory Care residents with ADLs and cognitive activities, using memory care techniques to stimulate function.

Qualifications

• Requires high school diploma or equivalent.
• Arizona Caregiver Certificate required.
• Strong communication and documentation skills.

Responsibilities

• Provide personalized assistance to Memory Care residents and support ADLs.
• Assist with grooming, meals, mobility, toileting, and hygiene per care plan.
• Monitor health changes, document observations, and report incidents to Health Services Director.
• Maintain resident documentation and charts, update Alert Charting as needed.
• Engage residents in social activities and support Life Enrichment Director.
• Follow Montessori Approach when applicable and keep work areas clean.
