Job summary

Vista Real Post Acute in Vista, CA is seeking a Full-Time LVN to provide direct nursing care to residents according to care plans and physician orders. The role pays $29.00 - $33.00 per hour and offers a stable full-time schedule.

You will administer medications, monitor conditions, document accurately, and support CNAs and nursing staff while collaborating with the interdisciplinary team to ensure quality outcomes.

Qualifications

• Current California LVN license in good standing.
• Current CPR/BLS certification.
• Skilled nursing or post-acute experience preferred, but new graduates are encouraged to apply.
• Strong communication and organizational skills.

Responsibilities

• Provide direct nursing care to residents in accordance with care plans and physician orders.
• Administer medications and treatments accurately and safely.
• Monitor resident conditions and promptly communicate changes to the clinical team.
• Maintain accurate and timely documentation.
• Supervise and support CNAs and other nursing staff as appropriate.
• Collaborate with interdisciplinary team members to ensure quality outcomes.
