The Primary Health Network is seeking a Licensed Practical Nurse for a full-time role in Sharon, PA. You will provide practical nursing care, support patient visits, and assist with medication and treatment needs within the LPN scope of practice.

Qualifications

• Graduate of an approved practical nursing program.
• Current, valid LPN license in PA and/or OH depending on location.
• Competency in practical nursing skills, medication administration, infection prevention, documentation, patient communication, and teamwork.

Responsibilities

• Provide nursing care per plan of care and provider orders.
• Room patients; document information and prepare for exams.
• Collect data, observe changes, and report findings to care team.
• Administer medications and therapeutic treatments as allowed by law and policy.
• Collect specimens and perform point-of-care testing.
• Document care and patient responses in the EMR.
• Reinforce patient education and assist with care coordination.
• Perform injections and venipuncture when privileges allow.
