SOUTHERN HEALTH PARTNERS INC in Wood County Jail, Wisconsin, is seeking a Full-Time LPN Medical Team Administrator to support our correctional healthcare team in Wisconsin. The role combines hands-on nursing with clinical coordination, team support, and daily healthcare operations to ensure patients receive consistent, compassionate care in a secure setting.

Registered Nurses (RNs) are encouraged to apply as well.

Qualifications

Active LPN license in Wisconsin.
Experience coordinating clinical activities in a healthcare setting.
Strong documentation and communication skills.

Responsibilities

Coordinate daily healthcare operations and help ensure quality patient care.
Provide nursing assessments, treatments, medication administration, and other clinical services within your scope of practice.
Support and coordinate healthcare team activities to promote efficient, consistent care.
Assist with patient care planning, chronic disease management, and follow-up services.
Maintain accurate medical records and ensure documentation meets established standards.
Communicate with healthcare providers, facility staff, and members of the medical team regarding patient needs.
Help maintain compliance with applicable healthcare regulations, policies, and clinical protocols.
Participate in on-call responsibilities to help address healthcare needs outside regular scheduled hours.
Promote a professional, respectful, and patient-centered healthcare environment.
