SOUTHERN HEALTH PARTNERS INC is seeking an LPN Medical Team Administrator to support our correctional healthcare team at Wood County Jail in Wisconsin. This on-site, full-time role combines hands-on nursing with clinical coordination and daily healthcare operations to keep care organized and patients well cared-for in a secure setting.

Eligible applicants include Licensed Practical Nurses, and RNs are encouraged to apply.

Responsibilities

• Coordinate daily healthcare operations and help ensure quality patient care.
• Provide nursing assessments, treatments, medication administration, and other clinical services within your scope of practice.
• Support and coordinate healthcare team activities to promote efficient, consistent care.
• Assist with patient care planning, chronic disease management, and follow-up services.
• Maintain accurate medical records and ensure documentation meets established standards.
• Communicate with healthcare providers, facility staff, and members of the medical team regarding patient needs.
• Help maintain compliance with applicable healthcare regulations, policies, and clinical protocols.
• Participate in on-call responsibilities to help address healthcare needs outside regular scheduled hours.
• Promote a professional, respectful, and patient-centered healthcare environment.
