Southern Health Partners is seeking a dedicated Licensed Practical Nurse (LPN) or Registered Nurse (RN) Medical Team Administrator to support our correctional healthcare team at Wood County Jail in Wisconsin.

This role combines hands-on nursing with clinical coordination, team support, and daily healthcare operations, with a Monday–Friday schedule and on-call responsibilities to ensure continuous patient care.

Qualifications

• Current, unrestricted LPN or RN license in Wisconsin or meeting state requirements.
• Current CPR certification or ability to obtain it.
• Strong organizational and communication skills; able to work independently with a team.

Responsabilités

• Coordinate daily healthcare operations and ensure quality patient care.
• Provide nursing assessments, treatments, medication administration, and related care.
• Support healthcare team activities and patient care planning.
• Maintain accurate medical records and ensure compliance with standards.

Ready to take the next step in your nursing career? Southern Health Partners (SHP) is seeking a dedicated Licensed Practical Nurse (LPN) Medical Team Administrator (MTA) to support our correctional healthcare team at Wood County Jail in Wisconsin!

This is an opportunity to combine hands-on nursing knowledge with clinical coordination, team support, and daily healthcare operations. You'll help keep care organized, support your fellow healthcare professionals, and ensure patients receive consistent, compassionate treatment in a safe and secure correctional setting.

What You'll Do

• Coordinate daily healthcare operations and help ensure quality patient care.
• Provide nursing assessments, treatments, medication administration, and other clinical services within your scope of practice.
• Support and coordinate healthcare team activities to promote efficient, consistent care.
• Assist with patient care planning, chronic disease management, and follow-up services.
• Maintain accurate medical records and ensure documentation meets established standards.
• Communicate with healthcare providers, facility staff, and members of the medical team regarding patient needs.
• Help maintain compliance with applicable healthcare regulations, policies, and clinical protocols.
• Participate in on-call responsibilities to help address healthcare needs outside regular scheduled hours.
• Promote a professional, respectful, and patient-centered healthcare environment.

Qualifications: What We're Looking For

• Current, unrestricted LPN or RN license in Wisconsin, or a license that meets applicable state requirements.
• Current CPR certification or the ability to obtain it as required.
• Strong organizational, communication, and clinical decision-making skills.
• The ability to work independently while collaborating effectively with a healthcare team.
• Dependability, sound judgment, and a commitment to quality patient care.
• Leadership, coordination, or supervisory experience is helpful.
• Correctional healthcare experience is welcome but not required!
