Qualifications include graduation from an approved LVN program, an active CA LVN license, and the ability to work independently. Nice-to-have: 2+ years LVN/LPN experience and bedside care recent practice.

Responsibilities

• Manage care for residents throughout the care continuum (pre-admission to post-admission).
• Coordinate services to meet residents' needs in a timely and effective manner.
• Collaborate with healthcare providers, families, and interdisciplinary teams to develop and adjust care plans.
• Monitor residents and ensure appropriate interventions are implemented.
• Facilitate smooth transitions of care with services and resources in place.
• Advocate for residents and families and provide education as needed.
