You will work with residents, families, and the care team to develop and implement care plans, ensure timely services, and advocate for residents to deliver high-quality, person-centered care.

Qualifications

• Must have LVN license to practice in California
• Must have graduated from an approved LVN program
• Ability to make quick judgments and work independently
• Knowledge of nursing practices, procedures, terminology, laws, regulations, and guidelines related to long‑term care

Responsibilities

• Oversee and manage delivery of care for residents from pre-admission through post-admission
• Coordinate services to meet residents' needs in a timely and effective manner
• Collaborate with healthcare providers, families, and interdisciplinary teams to develop and adjust care plans
• Monitor residents' progress and ensure appropriate interventions are implemented
• Facilitate smooth transitions of care with all services and resources in place
• Advocate for residents and families and provide education as needed
