Windsor Elk Grove Care and Rehabilitation Center in Elk Grove, CA seeks a Full-Time Case Manager (LVN/LPN) to lead care coordination for residents from pre-admission through post-admission. Qualifications

Must have: Graduated from an approved and certified LVN program.
Must have: Current LVN license to practice in the state.
Must have: Ability to make quick judgments and work independently.
Must have: Knowledge of nursing practices, procedures, terminology, laws, regulations, and guidelines pertaining to long-term care.
Nice to have: 2 years of LVN/LPN experience.
Nice to have: Active bedside care experience within the past three years.

Responsibilities

Oversee and manage the delivery of care for residents from pre-admission through post-admission.
Coordinate services to meet residents' needs in a timely and effective manner.
