Job summary

Discovery Commons Bethel Park, part of Discovery Senior Living, seeks a qualified LPN to deliver compassionate nursing care to residents in a vibrant senior living community. You will assess, document, administer medications, and collaborate with the care team to maintain safety and well‑being.

The role requires an active LPN license and at least two years of licensed nursing experience, ideally in assisted living or long‑term care. Nights, weekends, and flexible scheduling are required.

Qualifications

  • LPN license required, or RN per state regulations.
  • Minimum of two years of licensed nurse experience.
  • Experience in assisted living, long-term care, or skilled nursing preferred.
  • Strong clinical judgment and communication skills.

Responsibilities

  • Provide nursing care to residents per care plans and procedures.
  • Assess residents' physical and psychosocial status and monitor changes.
  • Administer medications and document appropriately.
  • Coordinate care with physicians, families, pharmacies and other providers.
  • Support health promotion and wellness initiatives within the community.
  • Ensure accurate and compliant clinical documentation.
  • Assist with care planning, updates, and resident evaluations.
  • Collaborate with caregivers and leadership to ensure quality care.
  • Respond to concerns and emergencies with sound clinical judgment.
  • Provide training and guidance for care staff as appropriate.
  • Ensure compliance with federal, state, and local regulations.
  • Participate in QA, audits, and background screening as needed.

As a Licensed Practical Nurse (LPN), you’ll play an essential role in enriching the lives of seniors by providing compassionate, high-quality nursing care while supporting resident safety, dignity, and well‑being every single day.

Position Highlights

  • Rate of Pay: $30/hour
What You’ll Do
  • Provide nursing care to residents in accordance with established care plans, policies, and procedures.
  • Conduct ongoing assessments of residents’ physical and psychosocial status and monitor for changes in condition.
  • Administer medications and treatments as prescribed and document appropriately.
  • Coordinate and communicate resident care needs with physicians, families, pharmacies, and other care providers.
  • Support health promotion, disease prevention, and wellness initiatives within the community.
  • Ensure accurate, timely, and compliant clinical documentation.
  • Assist with care planning, service plan updates, and resident evaluations as required.
  • Collaborate with caregivers, wellness team members, and leadership to ensure consistent, high‑quality care delivery.
  • Respond to resident concerns, incidents, and emergencies using sound clinical judgment.
  • Support training and guidance for care staff as appropriate.
  • Ensure the community adheres to applicable federal, state, and local regulations related to resident care and services.
  • Participate in quality assurance initiatives, audits, and compliance activities as needed.
  • Perform other duties as assigned.
Qualifications
  • Current Licensed Practical Nurse (LPN) license, or a Professional Registered Nurse as per state regulations.
  • Minimum of two (2) years of Licensed Nurse experience required.
  • Experience in assisted living, long‑term care, or a skilled nursing environment preferred.
  • Strong clinical judgment, problem‑solving, and decision‑making skills.
  • Effective organizational, time‑management, and communication skills.
  • Ability to manage multiple priorities and delegate appropriately.
  • Demonstrated commitment to seniors and person-centered care.
  • Ability to work nights, weekends, and flexible schedules as required by the community.
  • Proficiency with electronic documentation systems and basic computer applications.
  • Ability to respond to medical emergencies.
  • Exposure to bloodborne pathogens and infectious disease prevention.
  • Ability to lift or assist residents as required by care plans.
