POSITION SUMMARY

This per diem position offers an opportunity for the LVN to work under the direction of a Registered Nurse, providing skilled nursing and palliative care to patients in their homes. The LVN collaborates with other Interdisciplinary Team members, providing intermittent visits or shifts of skilled nursing care, including pain management, symptom control, and nursing observations. The LVN also educates patients, family members, or primary caregivers about the expected course of illness, care techniques, and other health measures. Participation in care plan development and implementation is required. Proficiency with computers and electronic medical records is necessary.

QUALIFICATIONS

• Graduate of an accredited vocational nursing program.
• Current LVN license in California. Certification in Hospice and Palliative Nursing is encouraged.
• At least one year of LVN experience.
• Valid driver's license, automobile insurance, reliable and well-maintained transportation.
• Spanish speaking ability is a plus but not required.
