Job summary

Hanford Home Health in Tri Cities, WA is seeking a Full Time Home Health Skilled Nurse (RN) to provide in-home, skilled nursing care for medically complex patients. The role reports to the Director of Clinical Services and involves 12-hour shift coverage with 36–40 hours per week.

Responsibilities include assessments, medication administration, plan-of-care delivery, and coordination with the clinical team. Must have WA RN license, CPR/BLS, and reliable transportation.

Qualifications

• Active Washington State RN license in good standing.
• Graduate of an accredited School of Nursing.
• Minimum of one (1) year of nursing experience; home health experience preferred.
• Current CPR/BLS certification for healthcare professionals.
• Washington State Food Worker's Card or ability to obtain upon hire.
• Valid driver's license, reliable transportation, and auto insurance.
• Ability to pass background check and drug screening.
• Ability to lift up to 50 lbs with proper body mechanics.

Responsibilities

• Provide skilled nursing care per Plan of Care and orders.
• Perform nursing assessments and ongoing monitoring within RN scope.
• Administer medications and treatments; monitor for side effects.
• Provide skilled interventions (e.g., wound care) as ordered.
• Measure and document vital signs, intake/output, pain, and weight.
• Report changes to supervising nurse or Director of Clinical Services.
• Educate patients and families on conditions, medications, and care requirements.
• Collaborate with interdisciplinary team to coordinate care.
• Assist with mobility, ADLs/IADLs as outlined in the Plan of Care.
• Document accurately in the electronic health record system.
• Follow infection control, safety, HIPAA, and confidentiality rules.
• Report suspected abuse or neglect per state law and policy.
• Comply with OSHA, Bloodborne Pathogens plan and PPE use.
• Complete required state continuing education.
• Perform only duties within RN scope and agency policies.
