Job summary

Sea Mar Community Health Centers is recruiting a Case Manager for BH Housing in Vancouver. You will oversee outreach efforts, provide peer counseling, assess housing needs, and develop action plans to secure stable housing for clients.

As part of Sea Mar’s mission, you will collaborate with housing and behavioral health teams, maintain accurate documentation, and ensure culturally competent care while upholding client confidentiality and safety.

Qualifications

• Housing or behavioral health related case management experience preferred.
• Ability to assess psychosocial, housing and economic needs of clients.
• Strong communication and interpersonal skills with diverse populations.

Responsibilities

• Coordinate outreach efforts and collaborate with external agencies.
• Provide peer counseling and motivational support to clients.
• Assess and develop housing rehabilitation plans to secure housing.
• Monitor documentation, reporting and PATH program data entry.
• Develop program policies and procedures with supervisor input.
• Ensure client-centered, culturally competent service delivery.
