Job summary

Sea Mar Community Health Centers in Tacoma is hiring a full-time Case Manager for the Tacoma Adult Treatment Center. The role works autonomously and within the Behavioral Health team, managing your own caseload and providing housing consultations to the mental health team.

You will participate in outreach, provide peer counseling, mentor clients, conduct psychosocial assessments, and assist with PATH data reporting and housing referrals; a valid driver's license and vehicle are required.

Qualifications

  • Bachelor’s degree in Counseling, Social Work, Psychology, or a related field.
  • Must be an Agency‑Affiliated Counselor in the State of Washington.
  • Must hold an active Certified Peer Support Certification in WA.
  • Must have access to a car and valid driver's license.
  • TB test at hire and annually; BLS CPR within 90 days.

Responsibilities

  • Provide housing consultation to the mental health team.
  • Design or refer housing interventions.
  • Participate in outreach and coordinate with outside agencies.
  • Provide peer counseling and mentor clients.

Case Manager (BH Housing) – Tacoma Adult Treatment Center

Hourly Rate: $22.85

Position Summary

Sea Mar Behavioral Health’s diverse and quickly expanding program seeks a Full‑time Case Manager for its Tacoma Adult Treatment Center. The Case Manager works autonomously and collaboratively as a member of the Behavioral Health team, participates in the design, implementation, and ongoing delivery of services, and is responsible for their own caseload, providing housing consultation to the mental health team, and designing or referring housing interventions.

Duties and Responsibilities
  • Participate in outreach services to prospective clients; coordinate outreach efforts with outside agencies and collaborate with coworkers and external personnel.
  • Provide peer counseling and support, drawing on common experiences as a peer, to validate clients’ experiences and encourage active participation in recovery.
  • Serve as a mentor to clients, promoting hope and empowerment.
  • Conduct ongoing psychosocial assessments of clients’ psychological, social, vocational, systemic, and economic goals and needs.
  • Monitor, compile, and submit data, reporting, and paperwork for the PATH program; assist clients in obtaining housing. Housing case‑management experience is a plus.

Other duties may be assigned as needed.

Education and Qualifications
  • Prefer a bachelor’s degree in Counseling, Social Work, Psychology, or a related field; work experience may be considered on an individual basis.
  • Must be an Agency‑Affiliated Counselor in the State of Washington.
  • Must hold an active Certified Peer Support Certification in the State of Washington.
  • Must have access to a car, a valid driver’s license, and current auto registration; proof of auto insurance and maintenance of minimum liability coverage as required.
  • Must obtain a current driver’s record meeting the agency’s standards and provide a copy of insurance endorsements to Human Resources.
  • Must pass a TB test at hire and annually.
  • Must obtain Basic Life Support (BLS) CPR within 90 days of hire and maintain certification.
