Job summary

AdvisaCare Home Health and Hospice seeks a compassionate MSW to join our hospice team in Grand Rapids, MI on a part-time/per diem basis. The ideal candidate has an MSW from a CSWE-accredited program and a passion for supporting patients and families dealing with life-limiting illness.

Preferred candidates include hospice experience and at least one year in home health care, plus a valid driver's license and reliable transportation.

• MSW degree from CSWE-accredited program.
• One year of Home Health Care or Hospice experience preferred.
• Hospice experience preferred.
• Valid Driver's License with reliable transportation.

• MSW degree from a school of social work accredited by the Council of Social Work Education (CSWE)
