Job summary

AdvisaCare Home Health and Hospice is seeking a compassionate MSW to join our hospice team in Grand Rapids, MI on a part-time/per diem basis. This role serves patients and families in the local area, with Troy as the parent agency.

The ideal candidate holds an MSW from CSWE, has hospice/home health experience preferred, and will provide support across the patient journey, with flexible scheduling and a supportive work environment.

Qualifications

• MSW degree from a CSWE-accredited program.
• One year of Home Health Care or Hospice experience preferred.
• Hospice experience preferred.

• MSW degree from a school of social work accredited by the Council of Social Work Education (CSWE)
