Job summary

Heart to Heart Hospice is seeking a dedicated Hospice Aide (CNA) to support patients under RN supervision. You will assist with transfers, ambulation, and essential daily care while maintaining confidentiality and a compassionate bedside manner.

Requirements include a current CNA license, a High School Diploma or GED, and at least one year of related experience. A valid driver’s license and reliable transportation are also required.

Qualifications

• High School Diploma preferred.
• Unencumbered CNA license current.
• At least 1 year experience as Hospice Aide or Nursing Assistant.
• EMR experience is a plus.
• Current driver’s license and reliable transportation required.

Responsibilities

• Provide direct patient care under RN direction per Plan of Care.
• Assist patient with safe transfers and ambulation per policy.
• Report and document changes to ensure continuity of care.
• Follow infection control principles.
• Promote respectful communication with patient and family.
• Submit documentation in a timely manner.
• Perform additional duties as needed.
• Ensure compliance with laws and policies.
