Job summary

AdvisaCare in Michigan is seeking a Registered Nurse for Home Health or Hospice care. You will provide compassionate, one‑on‑one nursing, perform comprehensive assessments, and coordinate individualized care plans with patients and families.

This role offers meaningful, flexible options (part-time or full-time) and the chance to serve communities across Traverse City, Kalkaska, Baldwin, Cadillac, Manistee, Gaylord, and nearby areas.

Qualifications

• Initial and ongoing assessment of patient needs (physical, functional, psychosocial, environmental)
• Implementation and revision of individualized care plans
• Ensure compliance with policies and Medicare requirements
• Educate patients/families on disease, self-care, end-of-life care, ethics
• Supervise paraprofessionals delivering services
• Prepare clinical/progress notes and participate in IDG conferences
• Maintain high customer satisfaction with clients, families and caregivers
• Communicate information using current processes and technology

Responsibilities

• Assess patients’ physical, functional, psychosocial, and environmental needs.
• Implement and update the care plan; monitor progress.
• Educate patients and families about disease processes and self-care.
• Supervise support staff delivering patient care.
• Document progress notes; participate in IDG conferences.
• Contribute to performance improvement initiatives and service quality.
