Job summary

Valley Regional Home Health is seeking a full-time Registered Nurse to cover Tulare, Visalia, Kingsburg, Selma, Dinuba, and Hanford areas. The role runs Monday through Friday with 8:00am–4:30pm training, then flexible start/end times as you render care in patients’ homes. Expect to see 3–5 patients per day and travel between visits.

Qualifications

• RN license to practice in the state
• Graduate of NLN-accredited nursing program
• Current CPR certification required
• Must have a valid driver’s license with reliable vehicle

Responsibilities

• Perform initial patient assessments and determine home care needs.
• Re-evaluate patient status and document care within required timelines.
• Initiate and revise the plan of care based on nursing diagnosis.
• Develop an individualized care plan with patient and family involvement.
• Administer medications and treatments as prescribed.
• Counsel patient and family on plan of care and health education.
• Provide discharge planning and coordination of services.
