Job summary

AccentCare is seeking a Physical Therapist for Home Health to provide evaluation, treatment, and education as part of an interdisciplinary team in the Lubbock area. This full-time role requires a licensed PT and CPR certification, with travel to nearby locations.

You will manage a caseload, document care, and collaborate with physicians and family members while meeting agency productivity standards. A CAPTE-approved master's degree and at least nine months' experience are preferred.

Qualifications

• Master's degree in physical therapy approved by CAPTE.
• Minimum nine months experience as a physical therapist.
• Licensed to practice in the state of operation.
• CPR certification currently valid.
• Licensed driver able to travel to all business locations.

Responsibilities

• Provides patient evaluation and treatment per physician orders and policies.
• Documents care and maintains required patient records.
• Collaborates with interdisciplinary team and family education.
• Manages caseload and meets productivity standards.
• Maintains current licensure and CPR certification throughout employment.
