Job summary

Qualifications

• Excellent communication with patients, families and team members.
• Fluent English reading/writing and clear documentation.
• Basic computer skills including Microsoft Office.
• Ability to refer patients to community resources effectively.
• Collaborates with interdisciplinary care teams.

Responsibilities

• Sees and plans home health aide activities in the absence of other healthcare professionals.
• Demonstrates effective communication with team members, patients, and families.
• Maintains clinical records per standards and timelines.
• Participates in orientation, in-service and staff development activities.
• Performs other duties as assigned.
