Job summary

Outcome HC in Georgia is seeking an Occupational Therapist to provide age-appropriate interventions in the patient’s home, collaborating with disciplines to meet rehabilitative goals.

The OT will deliver assessments, therapeutic exercises, education, and treatments, participate in staff education, attend inservices, and ensure regulatory compliance.

Requires NBCOT, GA OT license, and at least 1 year of full-time OT experience; strong communication and organizational skills preferred.

Qualifications

• Graduate of an accredited school of Occupational Therapy.
• NBCOT and Georgia State Board licensure required.
• Minimum 1 year of full-time OT experience preferred.
• Strong critical care assessment and rehabilitative skills.
• Experience with home health operations and regulatory processes.

Responsibilities

• Direct patient care: assess and manage rehabilitative needs from admission to discharge; meet productivity standards; seek leadership guidance as needed.
• Coordination of services: develop and revise the plan of care; coordinate with Clinical Supervisor and team; perform other duties as assigned by the Director of Therapy.
• Patient education: serve as educator; report concerns affecting patient safety; use verbal and written tools to promote independence.
• Staff education and professional development: participate in orientation; attend continuing education; stay current in clinical knowledge.
• Communication & documentation: complete skilled intervention documentation on time; report status changes; adhere to regulatory guidelines; participate in care team conferences; check messages daily.
• Regulatory guidelines: apply CMS/State and Joint Commission guidelines; complete documentation in EMR within timeframes.
