ACE Home Health Care & Hospice is seeking full-time License Vocational Nurses (LVN) for home health care in Alameda County, from Hayward to Berkeley.

Minimum Requirements: At least one year of experience in a home health agency or community health setting.

Main Duties & Responsibilities

• Assess and develop a care plan based on patient needs, physician orders, and agency policies.
• Assist in ongoing patient assessments to implement the care plan.
• Maintain home charts during each assignment following clinical/nursing procedures.
• Initiate preventive and rehabilitative nursing procedures.
• Counsel and instruct patients and families to meet care needs.
• Complete and submit timesheets and clinical notes accurately and timely.
• Serve as liaison between agency, patient, and physician as needed.
• Communicate clearly with patients, supervisors, and physicians, reporting changes and findings promptly.

Knowledge & Skills: Clinical knowledge, critical thinking, excellent interpersonal and customer service skills, problem-solving abilities, and knowledge of Medicare, Medi-Cal, State, and Federal regulations.
