Position Overview

ACE Home Health Care & Hospice is seeking full-time Licensed Vocational Nurses (LVN) for home health care in Alameda County, from Hayward to Berkeley.

Minimum Qualifications

Minimum of one year experience in a home health agency or community health setting.

Main Duties & Responsibilities
  • Assess and develop a care plan based on patient needs, physician orders, and agency policies.
  • Assist in ongoing patient assessments and implement the care plan.
  • Maintain home charts according to clinical/nursing procedures.
  • Initiate preventive and rehabilitative nursing procedures.
  • Counsel and instruct patients and families.
  • Complete and submit timesheets and clinical notes accurately.
  • Act as liaison between agency, patient, and physician.
  • Communicate effectively with patients, supervisors, and physicians, reporting changes promptly.
Knowledge & Skills

Strong clinical knowledge, critical thinking, excellent interpersonal skills, and understanding of Medicare, Medi-Cal, and federal regulations.
