Transitions At Home for Central Wisconsin is seeking a Full-Time Licensed Practical Nurse (LPN) to join our compassionate team serving Montello. The LPN provides skilled nursing care in patients' homes, manages medications, coordinates services, and counsels patients and families under the RN Clinical Manager.

Hours are 40 per week, with no night shifts or weekends and limited on-call. Competitive wages and benefits are offered to eligible employees.

Formación

• Two years' nursing experience, 1 year in Home Health preferred.
• CPR Certification.
• A valid Wisconsin Driver's License and Car Insurance.

Transitions At Home, is currently seeking to hire a Full-Time compassionate and dedicated Licensed Practical Nurse (LPN) to join our team and provide exceptional care to our patients in the Montello area.

What does a Transitions at Home LPN do?

Transitions At Home Licensed Practical/Vocational Nurse provide skilled nursing care to include, routine visits, medication management, treatments, and coordinates services, counsels the patient and family/significant others in their place of residence in accordance with the patient’s physician orders and plan of care under the direction and supervision of the RN Clinical Manager.

Hours: Full-Time

• 40 hours per week 8:00 am - 4:30pm
• Monday through Friday
• No Night Shifts or Weekends
• Limited on-call rotation

Qualifications / Job Requirements:
