BrightStar Care in Rochester, MN is seeking a dedicated Home Care Field Specialist to join our team. This full-time role provides high-quality home care to clients with diverse and complex needs, with a focus on companionship, daily activities, and mobility support.

You will travel within a 35-mile radius of Rochester, maintain a reliable vehicle, and hold a valid driver’s license and auto insurance. Expect flexible scheduling across shifts with guaranteed 30 hours per week and a requirement for

Responsibilities

• Provide companionship and conversation to enhance the lives of our clients.
• Assist with daily activities such as meal preparation, light housekeeping, and errands.
• Provide physical assistance such as bathing, dressing, grooming, transferring, and ambulation.
• Accompany clients to appointments or social outings, providing transportation as needed.
• Monitor and report changes in the client’s health, behavior, and needs.
• Act as a member of the Care coordination team to communication with the RN Case Manager to update the plan of care.

Job description

We are currently seeking a dedicated and adaptable Home Care Field Specialist to join our team. This full-time role requires providing high-quality home care services to clients with diverse and often complex needs. The ideal candidate must be comfortable working with Hoyer lifts and have reliable transportation, valid drivers license and current auto insurance. This position offers 30 guaranteed hours per week, with a required 40-hour weekly availability to ensure flexible scheduling and coverage. Must be comfortable traveling within a 35 mile radius of Rochester as we service clients in Rochester and the surrounding cities.

• Premium Short Shift Pay: We offer a variety of shift Lengths ranging from 1 hour to 12 hours. Our 1-hour shifts get paid at $30 per hour and hour 2-hour shifts get paid at $22.50 an hour. Any shift 3 hours or longer is paid at a base rate between $19-$22 based on experience.

Requirements:

• Experience providing care to clients in a hands-on environment (Home Health, Assisted Living, Group Home, Hospital Setting)
• Reliable vehicle and a valid driver’s license with Auto Insurance.
• Negative TB test or Cleared Chest-Xray.
• Availability to work 2 weekend days per month.
