The Heal & Revive Chiro Clinic is seeking a dedicated and compassionate Physical Therapist to join our dynamic team. In this full-time position, you will provide essential physical therapy services directly in patients' homes, focusing on creating personalized care plans that enhance mobility and reduce pain. You will play a critical role in helping clients regain independence and improve their quality of life through innovative, evidence-based therapy techniques.

Work Environment

This position is entirely onsite, with Physical Therapists traveling to patients' homes across New York City. You will have the autonomy to evaluate and treat clients in their environments, allowing you to observe their daily challenges and tailor interventions accordingly. As part of an interdisciplinary team, you will collaborate with other healthcare professionals to provide comprehensive and cohesive care.

Key Responsibilities

• Direct physical therapy treatment to prevent disability, alleviate pain, and develop or restore muscle function based on individualized care plans.
• Conduct thorough evaluations, including diagnostics and functional assessments to determine the clients' specific needs and abilities.
• Oversee and guide Physical Therapy Assistants in the delivery of care, ensuring adherence to treatment protocols.
• Formulate detailed home rehabilitation plans that consider environmental factors and aim to enhance client independence.
• Administer hands-on therapeutic techniques such as manual therapy, and instruct clients in exercises and functional activities that build strength and mobility.
• Ensure accurate and timely clinical documentation, including evaluations, care plans, and progress notes.
• Monitor clients' responses to therapy, reporting significant changes to physicians and adjusting treatment plans as necessary.
• Engage with clients and their families, providing education on therapy devices and self-care techniques to support their rehabilitation journey.
• Participate in case conferences to develop comprehensive care strategies in collaboration with an interdisciplinary team.
• Advocate for patient-centered practices, representing the clinic's values and mission at all times.

Patient Population / Client Focus

Our patient population includes individuals recovering from surgery, managing chronic pain, elderly clients needing mobility support, and those requiring rehabilitation after injury or illness. You will work closely with diverse demographics, addressing their unique physical and emotional needs while fostering a supportive and encouraging environment.

Qualifications

• Doctorate in Physical Therapy from an accredited program.
• Valid PT license in good standing in the state of New York.
• Current CPR certification.
• Strong communication skills and ability to engage effectively with patients and their families.
• Effective problem-solving skills with the capacity to make sound clinical decisions.

Preferred Experience

A minimum of one year's experience in physical therapy, preferably in acute care or home care settings, is highly desirable. Familiarity with OASIS documentation and interdisciplinary team collaboration will enhance your contributions in this role.

We offer a competitive annual salary ranging from USD 90,000 to 100,000, commensurate with experience.
