Responsabilités

• Screen and evaluate patients with cognitive and/or ADL disorders and develop plans of care.
• Utilize standardized assessments and evidence-based practice to support clinical interventions.
• Provide rehabilitative, skilled treatment per plan and regulatory standards.
• Document treatments and progress in the clinical record; adjust plans as needed.
• Communicate with patients, families, and health team about progress and discharge summaries.
