Job summary

Evolve Therapy Services is seeking a compassionate Physical Therapist to support an in-house Skilled Nursing Facility in Suring, Wisconsin. You will evaluate residents, develop treatment plans, and collaborate with an interdisciplinary team to ensure high-quality care.

This role involves documentation in NetHealth EMR, adherence to CMS standards, and ongoing professional development. Flexible PT schedules support full-time, part-time, or PRN engagements.

Qualifications

• BS in Physical Therapy from an accredited program
• Active Physical Therapy license in state(s) of practice
• Ongoing continuing education to maintain licensure and certification
• Commitment to ongoing professional development to maintain licensure and certification

Responsibilities

• Provide physical therapy services to residents according to established standards and procedures.
• Document and bill therapies per CMS and company procedures.
• Evaluate and plan resident therapy programs with goals and timelines.
• Screen residents regularly to determine intervention needs.
• Screen referrals promptly to determine intervention or treatment needs.
• Conduct evaluations to gather data for treatment planning.
• Perform specialized evaluations as needed.
• Develop treatment plans with short and long-term goals.
• Communicate evaluation findings and plans with residents, families, and interdisciplinary team.
• Complete compliant documentation in NetHealth EMR systems
