Job summary

Northeast Arc is seeking a Licensed Practical Nurse (LPN) to join our residential care team in Topsfield, MA. The LPN provides direct care, follows the care plan, monitors medical status, and reports to the on-call RN, collaborating with staff to support residents in their community residence.

Responsibilities include health monitoring, medication administration, care plan implementation, and medical coordination while ensuring compliance with MAP regulations and safety standards.

Qualifications

• Licensed LPN in Massachusetts with minimum 1 year of experience.
• Experience working with individuals with intellectual or developmental disabilities preferred.
• Must maintain CPR certification and complete required trainings.
• Must have a valid driver's license and reliable transportation.

Responsibilities

• Conduct health assessments and monitor medical status on shift.
• Administer medications and document per MAP regulations.
• Demonstrate competence in trach and vent care and related emergencies.
• Prepare and submit daily data, incident and progress notes.
• Coordinate medical appointments and communicate with the team.
• Uphold universal precautions and maintain licenses.
