Job summary

Ultimate Therapy Services is hiring a full-time Certified Occupational Therapy Assistant at Autumn Lake at Madison in Madison, CT. You will work with skilled nursing residents, following the supervising OT's plan of care, to rebuild skills for daily activities.

Responsibilities include implementing therapy interventions, guiding residents through self-care tasks, and tracking progress with accurate notes. CT licensure and COTA certification are required; new grads welcome.

Qualifications

• Active CT Occupational Therapy Assistant license and COTA certification.
• Ability to follow treatment plans with attention to resident safety and comfort.
• Strong communication, organization, and documentation skills.
• New graduates welcome to apply; experience in skilled nursing is a plus.

Responsibilities

• Carry out individualized interventions in the OT's plan of care.
• Help residents with dressing, bathing, grooming, and toileting with cueing as needed.
• Lead prescribed exercises addressing upper-extremity movement and tolerance.
• Teach adaptive equipment techniques and energy conservation per OT guidance.
• Track progress and communicate changes to the supervising therapist.
• Complete accurate treatment notes and collaborate with nursing staff.
