Job summary

Continuum Rehab Group is seeking a licensed COTA to join our SNF therapy team in Selma, CA. This full-time role supports Occupational Therapy services and collaborates with the OT, nursing, and rehab staff to achieve resident goals.

You will assist in evaluations, treatment planning, and direct patient care, helping residents regain independence and quality of life in a supportive environment.

Qualifications

• Licensed as a Certified Occupational Therapy Assistant (COTA) under state requirements.
• Experience assisting in evaluations, treatment planning and direct patient care.
• Familiarity with Medicare/Medicaid reimbursement systems may be beneficial.

Responsibilities

• Assist OT in daily operations of therapy services in a SNF.
• Support treatment planning and implementation to meet resident goals.
• Provide education and training to patients and caregivers.
