Job summary

Willow Creek Health Care Center in California seeks a full-time Certified Occupational Therapy Assistant (COTA) to join our rehabilitation team. Under supervision of an Occupational Therapist, you will provide direct patient care and support residents in achieving independence.

Responsibilities include implementing treatment plans, documenting progress, coordinating discharge planning, and educating residents and families.

Qualifications

• Graduate of an accredited Occupational Therapy Assistant program.
• Current, active, and unencumbered COTA license in the State of California.
• Experience in a skilled nursing or post-acute setting preferred.

Responsibilities

• Provide skilled occupational therapy treatments as directed by the supervising Occupational Therapist.
• Carry out therapy interventions in accordance with physician orders and established treatment plans.
• Document daily treatment notes, weekly progress notes, and other required records in compliance with state practice acts and payer regulations.
• Communicate resident progress, concerns, and treatment updates to the Occupational Therapist and interdisciplinary team.
• Assist with discharge planning and caregiver education.
• Instruct residents, families, and nursing staff in maintenance programs and safety techniques.
• Participate in interdisciplinary care conferences as needed.
• Maintain cleanliness and organization of therapy areas and equipment.
• Report equipment issues to ensure safe and effective operation.
• Maintain professional and respectful communication with residents, families, and team members.
