Job summary

Wasatch Behavioral Health in Provo, Utah, is seeking a full-time Case Manager at our South Provo Clinic. This on-site role pays $21.36-$29.88/hour and focuses on adults with mental illness, helping with daily living, housing, and accessing community resources.

Schedule Monday–Friday, 8:00 AM–5:00 PM. You will assess clients, develop care plans, coordinate care with therapists and other providers, maintain records, and advocate for client independence.

Qualifications

• One of the following educational paths is required: Associate's degree or equivalent with pursuing SSW licensure; Bachelor's degree with SSW license; Bachelor's degree with willingness to pursue SSW licensure.
• Valid driver's license and basic understanding of Social Work principles.
• Experience in social services or case management is beneficial.
• Applicants must be able to work onsite in Provo, Utah.

Responsibilities

• Conduct client assessments and gather essential background information.
• Develop individualized care plans based on client needs and treatment goals.
• Provide ongoing support, advocacy, and resources to help clients achieve stability and independence.
• Help clients access community resources, housing assistance, benefits, and healthcare services.
• Coordinate care with clinicians, therapists, nurses, prescribers, and other providers.
• Maintain accurate documentation, case notes, and client records.
• Collaborate with onsite team and community partners to support clients and clinicians.
• Build rapport with clients through compassionate, client-centered support.
• Ensure compliance with program policies and licensing requirements.

Education

Associate's degree or equivalent and pursuing SSW licensure

What You'll Do as a Case Manager

• Coordinate care with clinicians, therapists, nurses, prescribers, and other healthcare providers.
• Build strong rapport with clients while providing consistent, compassionate support.
• Maintain accurate documentation, case notes, and client records.
• Collaborate with the onsite team and community partners to support clients and clinicians.

Qualifications

Applicants must meet ONE of the following:

Note: Applicants under the "working toward SSW" pathway must obtain their SSW license within 24 months of hire.

• Associate's degree or equivalent and working toward SSW licensure
• Bachelor's degree with Social Service Worker (SSW) license
• Bachelor's degree in any field with willingness to pursue SSW licensure

Additional Requirements

• Valid driver's license
• Basic understanding of Social Work principles
• Familiarity with social welfare policy and human development
• Ability to problem-solve and maintain a client-centered approach
• Spanish speakers may qualify for increased wages after testing

Employment at Wasatch Behavioral Health is contingent upon successful completion of a drug screen, motor vehicle record review and criminal background check. After hire you must complete and receive a Criminal Background Clearance (BCI) from Utah State Office of Licensing. Upon receipt of a conditional offer of employment in the position, you must provide an official copy of licensure or certification.
