Job summary

Wellvance Community Mental Health Authority is seeking an ACT Case Manager to support individuals with severe mental illness in managing symptoms and achieving better community living. You will work within the ACT fidelity framework and Wellvance policies to deliver high-quality care on-site in Michigan.

The role requires a Bachelor’s degree in a human services field, at least one year in mental health, and Michigan licensure/certification.

Qualifications

• Bachelor’s degree in a human services field.
• One year of experience in the mental health field.
• QMHP, QIDP, CMHP, or Michigan licensure in the field.
• Ability to travel within our service area.
• Valid Michigan driver’s license and vehicle insurance.

Responsibilities

• Assist individuals with severe and persistent mental illness to manage symptoms and improve quality of life.
• Help clients integrate into the community per ACT fidelity model and Wellvance policies.
• Participate in on-call rotation as needed.
