About the role We are looking for a Full Stack Engineer who can ship features end-to-end across the platform without waiting for someone else to fill in the gaps. You will pick up work that touches the frontend, backend, database layer, and sometimes the infrastructure underneath it all. You will write code that goes into production, and you will be expected to do that at a pace that only works when AI tooling is part of how you think rather than a thing you reach for occasionally.
The platform is a HIPAA-compliant, cloud-native EHR built FHIR-native from the start. The stack is TypeScript end-to-end, runs on AWS, and is designed for AI-assisted development from the inside out. Hono on Lambda for the serverless backend. React 19 with Vite for the SPA. Drizzle for the ORM. tRPC paired with REST and FHIR for the API layer. Every choice in the stack is meant to get the most out of AI code generation, and the team ships at a pace that is only possible when the first draft of code coming out of Claude or Cursor actually compiles and runs. You will work closely with the Lead Product Architect and the rest of the engineering team. The architecture is set. Your job is to extend it cleanly, write code that fits the patterns already in the codebase, and push back when something does not feel right rather than silently working around it.
Preferred EHR or healthcare experience. Prior work on an EMR, EHR, or system that integrates with one. Bonus if you have shipped in post-acute, skilled nursing, or assisted living.
FHIR R4. Hands‑on experience with FHIR R4 resources, bundles, and search parameters. Bonus if you have built a FHIR native system rather than a FHIR facade over a legacy schema.
Modern TypeScript stack. Production work with Hono, tRPC, Drizzle ORM, or comparable modern TypeScript tooling. We can ramp up on the specific tools, but prior fluency accelerates everything.
Infrastructure as code. AWS CDK in TypeScript is what we use. Terraform, Pulumi, or comparable production experience translates.
Multi‑tenant SaaS. Production experience with PostgreSQL row‑level security, pgPolicy, or comparable tenant isolation patterns. Healthcare multi‑tenancy is a stronger signal still.
Testing depth. Vitest, Playwright, fast check, Testcontainers, or comparable tooling at each layer of the testing pyramid. You know how to write tests fast enough that the team actually runs them.
OpenTelemetry, Sentry, Pino, or comparable structured logging and tracing tools inside a serverless system.
Open source contributions. Visible work in public repositories, especially in the TypeScript ecosystem.
This role is based in Brookfield, Wisconsin, and will eventually require in‑office presence. Remote work is negotiated on a case‑by‑case basis. We work with engineers across time zones.
Contract to hire. If you join on a contract‑to‑hire basis, we expect the work to be mostly remote during the contract period, with a transition to on‑site work within six months.
Time zone overlap. You should be able to overlap by a few hours with US Central time on most working days. Standups, design discussions, and review conversations happen during US business hours.
Occasional travel may come up for integration testing or a team gathering. This is not a frequent travel role.
On‑call. You participate in the on‑call rotation. The system runs in real facilities, and engineering ownership extends to production reliability around the clock.
Camera‑on policy. We are a remote team, and we hold our online meetings on camera. Cameras stay on by default for internal meetings, customer calls, and stakeholder reviews. Real‑time presence matters when the team is distributed, and the work depends on reading the room.
Reporting structure. Reports to the Lead Product Architect. Partners daily with the rest of the engineering team and with product.
