Job summary

RiverVista Behavioral Hospital in Dayton, OH is seeking an Intake Assessor (RN or LSW) for full-time Day/Night shifts. Compensation is $28-38 per hour with a $10,000 sign-on bonus.

The role involves 3x12s shifts with every other weekend rotation, and duties include fast, trauma-informed evaluations to guide appropriate care. Responsibilities include completing biopsychosocials, risk screens, coordinating bed placement, and collaboration with on-call psychiatrists.

Qualifications

• Active independent licensure as RN or LSW.
• At least 1 year of acute behavioral health or crisis evaluation experience.
• Experience with risk assessment and de-escalation.
• Familiarity with payer authorization processes.
• Ability to document clearly in EMR and coordinate bed placement.

Responsibilities

• Complete biopsychosocial assessments, suicide and violence risk screens (e.g., C-SSRS), and ASAM when indicated.
• Obtain collateral, verify benefits, and coordinate bed placement with nursing/medical leadership.
• Consult with the on-call psychiatrist/NP; formulate provisional diagnoses and recommendations.
• Document clearly in the EMR and communicate decisions to patients, families, and referral sources.
• Ensure continuity of care overnight with vigilant observation and timely documentation.
• Manage after-hours admissions and urgent needs; prepare a thorough handoff to the day team.
