Job summary

ClearVista Behavioral Hospital is seeking an RN Assessor to coordinate care with interdisciplinary teams and external providers, participate in treatment planning, and ensure timely documentation. The role includes providing warm referrals and managing admissions verifications.

Night shift, 7pm–7am, with option for Full Time, Part Time, or PRN in Lorain, OH. Sign-on bonus of $7,000 is offered; RN licensure required and education options include RN or MSW/L icensed counseling track.

Qualifications

• Bachelor's degree (LSW or RN preferred), or Master's degree in social work or counseling with licensure.
• Must be 21 yrs or older.
• RN (Registered Nurse) required

Responsibilities

• Coordinates care with interdisciplinary teams and external providers and updates treatment team on clinical information.
• Participates in treatment planning, including psychosocial assessments.
• Provides resource information and "warm-referrals" to community providers for engagement.
• Completes thorough, timely documentation in compliance with standards.
• Demonstrates crisis intervention skills and upholds confidentiality.
• Completes the admission process, including verification of benefits and necessary authorizations.

Shift:

• 3 12-hour shifts per week


• Full Time, Part Time, and PRN (Full Time: 3 12-hour shifts, with every other weekend rotation)


• Night Shift

Hours: 7p-7a
