Responsibilities

Job Summary

The Registered Nurse in Home and Community-Based Services provides and directs nursing and personal care to patients in their homes as prescribed by the physician or as authorized by the state/program, in compliance with applicable laws, regulations, and agency policies. The RN coordinates the care plan with other healthcare professionals and helps maintain continuity of care by planning and exchanging information with physicians, agency personnel, patients, families, case managers, and community resources.

Specific Job Duties/Responsibilities

• Provides high-quality clinical services within scope of practice and infection control standards, in accordance with the plan of care/service plan, and in coordination with other care team members from admit through discharge.
• Completes clinical nursing assessments as required by federal and/or state program requirements and payers.
• Ensures patient eligibility and medical necessity for services as defined by payer source and agency policy.
• Develops and revises individualized care and service plans according to federal and/or state requirements with community providers.
• Ensures plan of care meets patient needs and initiates revisions as needed with physician approval, complying with regulations.

Education and Experience

Experience Desired

• A minimum of one year experience as an RN preferred.

License Requirements

• Current RN licensure in the state of practice.
• Valid driver's license, vehicle insurance, and access to reliable transportation.
• Current CPR certification required.
