Job summary

Serenity Hospice is seeking a dedicated Hospital On-Call RN to support our patients across Essex County, NJ. The role covers hospital locations within the Northern Hospital Division during on-call hours, with shifts evenings and every other weekend.

Responsibilities include admissions, Medicare-eligibility reviews for GIP, and on-call follow-ups to ensure needs are met, collaborating with physicians, nurses, social workers, and families to deliver compassionate end-of-life care.

Qualifications

• Current NJ RN License in good standing.
• Strong communication and interpersonal skills.
• Hospice or hospital experience preferred but not required.
• Willing to travel to assigned hospitals with reliable transportation and liability insurance.

• Complete evaluations, admissions, and discharges within our Northern Hospital Division as needed.
• Medicare-eligibility review for new referrals and determining if patients meet criteria for GIP-level hospice.
• Follow up on current GIP hospice patients to ensure needs are met during on call hours.
• Reinforce hospice recommendations made by RN Case Managers and make adjustments as needed.
• Collaborate with doctors, nurses, social workers, patients, families, and our hospice staff to promote exceptional end-of-life care.
• Be available to answer any questions and provide education as needed regarding hospice services and philosophy.
