Job summary

Aurora Valley Care Skilled Nursing & Rehab, a Caldera Care facility, is seeking an Licensed Practical Nurse (LPN) for evening and night shifts. This full-time role offers competitive pay and the chance to provide high-quality resident care on a dedicated unit.

Responsibilities include administering medications, completing nursing assessments, and coordinating with the care team. An active LPN license, CPR, and state requirements are required.

Qualifications

• Current and valid LPN license.
• Multistate license with the 6-Hour Suicide Prevention Certificate.
• Excellent interpersonal skills.
• Fast and adaptive problem-solving abilities.
• CPR and First Aid certifications.

Responsibilities

• Provide nursing care to patients on the assigned unit.
• Administer and record medications and treatments as ordered.
• Complete nursing assessments, progress notes, and charting.
• Educates residents and families on health care instructions.
• Collaborates with the care team to plan, implement, and evaluate nursing care.
• Assists in the admission, transfer, and discharge of patients as necessary.
• Communicate with Providers and nurse management regarding patients' condition, changes, medications, vitals, etc.
• Provide direct care to residents as needed.
• Direct unit staff as necessary.
• Report to direct supervisor (Unit Nurse Manager) all resident and staff issues/concerns as needed.

Pay Range: $38-$43 DOE

FT Position

Licensed Practical Nurse (LPN) Duties and Responsibilities:

• Provide nursing care to patients on assigned unit
• administer and record medications/treatments as ordered
• Complete nursing assessments, evaluations, progress notes, and alert charting as necessary
• Educates and provides health care instructions to residents/families as necessary
• Collaborates with other team members in planning, implementing, and evaluating nursing care and other patient activities
• Assists in the admission, transfer, and discharge of patients as necessary
• Communicate with Providers and nurse management regarding patients' condition, changes, medications, vitals, etc.
• Provide direct care to residents as needed
• Direct unit staff as necessary
• Report to direct supervisor (Unit Nurse Manager) all resident and staff issues/concerns as needed

Licensed Practical Nurse (LPN) Qualifications and Skills:

• Current and Valid LPN license

• In-depth understanding of patient care policies and regulations
