Hourly Pay Rate:$20.00
Part-Time Schedule: Between Monday – Friday; 8 AM – 5 PM (varies depending on client needs)
Location: Mesa, AZ
DUTIES AND RESPONSIBLITIES. Responsibilities will include but not limited to:
- Maintaining caseload of licensed foster families.
- Complete accurate case documentation & agency paperwork in a timely and correct manner.
- Coordinate services with clients/families and other service providers.
- Conduct monthly home visits for foster families with placements in the home & Quarterly home visits for foster families with no placements in the home.
- Participate in the Child Family Team (CFT) meetings, attend court with the family, participate in Team Decision Meetings (TDM) and other structured engagements with the Department, system partners, birth parents and other activities that support the permanency goal for each child.
- Intercede for the family to reduce conflict or stress between the family and any parties involved.
- Document in QC any communication with entities other than DCS regarding the child in care within ten (10) business days, including but not limited to: the date of contact, name and role of the person contacted, and summary of contact.
- Assure compliance of each foster provider with program policies and state licensing or certification requirements
- Drive personal vehicle to conduct program business
- Access Community Resources for obtaining assistance when needed.
- Educate the family on reasons why some decisions might be made
- Seek resolutions by problem-solving in partnership with the family and elevating concerns when appropriate
- Remain impartial and objective
- Participate and attend staff training as needed (as relevant to program/client needs).
- Complete amendments and renewals within specified timeline and following procedural requirements.
- Ensure compliance of provider files in Extended Reach.
- Attending scheduled program events.
- Assist in the translation of barriers and successes into positive system changes (as relevant to program/client needs).
- Support utilization management functions as needed in conjunction with supervisor.
- Attend external agency meetings as needed to assist your supervisor.
- Assist in improving ongoing program development.
- Drive personal vehicle to conduct program business
- Assist in writing initial home studies if needed.
- Maintain client demographic and licensing information and report this information to Program Manager & Program Coordinator as needed.
- Ability to work within the required programs and technologies needed to perform job duties.
- Complete required reports as needed.
QUALIFICATIONS
Education:
- High School Diploma or GED required.
- Bachelor's Degree in Behavioral Health, Social Work, or Human Services preferred.
Experience:
- A minimum of one (1) year of experience working with caregivers or working in child welfare.
- This can be a combination of education, experience and completed the training necessary for the type of service they are delivering.
- Ability to relate well with families of varied lifestyles, backgrounds, and ethnicities
- Experience in cultural diversity
- Ability to assess a family’s strengths and needs
- Experience in medication and other interventions
- Ability to recognize abuse and/or neglect in the home
Compliance:
- Relias Training required as assigned
- Class One Fingerprint Clearance Card
- Valid Driver’s License with reliable transportation
- Current Auto Liability Insurance
- 39 Month Motor Vehicle Record
- Must Be At Least 21 Years of Age
