Cornerstones of Care in Kansas is seeking an Integrated Eligibility Coordinator to perform federal benefits eligibility assessments and coordinate tasks with DCF and KLS partners. You will enter information into the EHR, manage school data entry, and support placement changes for youth in out-of-home care.

The role requires 1–3 years of related experience, a high school diploma or GED, and a valid driver’s license.

Formación

• 1–3 years of relevant experience in a coordinating/clerical role.
• High school diploma or GED required; associates preferred.
• Pass background check, physical, and drug screening; valid driver's license.

Responsabilidades

• Provide administrative and clerical support to the Director of Integrated Care Management.
• Support education for children in out-of-home care and coordinate records.
• Maintain medical, education, and mental health records in the EHR.
• Prepare documentation for federal benefits processes and enrollments.
• Input data in the EHR and track data in spreadsheets; may facilitate BID meetings.
